The district nurses are eager to cooperate with you to ensure that your child is healthy and ready to learn.  They travel from school to school but are always available by cell phone if an emergency or questions arise.  They are usually at your child’s school 2 days a week, except during mass screenings or teachings.

Health History Form:  Should be completed annually for all students, usually done at the enrollment center.  The information you provide on this form will be used to alert us to any health conditions your child may have.  Please make sure to keep our records current by sending in writing if your child develops any new medical conditions/issues or if medications change during the school year.  Please also alert the school to any changes in contact information that may occur during the school year.  Our records can only be as accurate as the information we receive.  Keeping up to date information will help us to better serve you and your child.  The form can be found on the Forms page.

  • Please note, for students with allergies, asthma, diabetes, hemophilia, or seizures, additional information may be needed to keep your child safe while at school.

In case of Accident/Injury/Illness:  The health offices are stocked with basic first aid supplies.  Cuts and scrapes will be cleaned with soap and water.  You will be notified if your child is ill and needs to go home.  If your child has fever, he/she needs to be fever free for 24 hours without the use of fever reducing medication before he/she may return to school.  If your child has diarrhea and or vomiting, there should be 24 hours with no further vomiting/diarrhea before he/she returns to school.  If your child needs to see a doctor for the illness, please bring notice from the doctor’s office regarding this.

Medications in the Health Office:  All medications at school will be kept locked in a central location.  All medications will need proper consent forms signed by parents/guardians and in most cases by the physician.

If An Emergency Occurs:   It is necessary to inform the health office of telephone numbers where parents may be reached if an emergency arises. We also request names and phone numbers of 2 reliable friends or relatives who could assume temporary care of your child if an illness or an accident should occur and you cannot be reached.

If a parent/guardian or emergency contact person cannot be reached or is unavailable to pick up a student that needs medical attention, EMSA may be called. Any expenses accrued will be the responsibility of the parent/guardian.
