Immunizations

Any students not meeting immunization requirements will be excluded from school.

OKLAHOMA STATE REQUIREMENT FOR SCHOOL ENTRY (PREK)

  • 4 doses of Diphtheria, Tetanus & Pertussis vaccine (DTaP)
  • 3 doses of Polio vaccine (IPV, OPV)
  • 1 doses of Measles, Mumps, Rubella vaccine (MMR)
  • 3 doses of Hepatitis B (Hep B)
  • 2 doses of Hepatitis A (Hep A)
  • 1 dose of Varicella (Chickenpox)

OKLAHOMA STATE REQUIREMENTS FOR SCHOOL ENTRY (K-6TH)

  • 5 doses* of Diphtheria, Tetanus & Pertussis vaccine (DTaP)
    • *if 4th dose is administered on or after the child’s 4th birthday, then the 5th dose is not required
  • 4 doses* of Polio (IPV, OPV)
    • *if 3rd dose is administered on or after the child’s 4th birthday, then the 4th dose is not required
  • 2 doses of Measles, Mumps, Rubella vaccine (MMR)
  • 3 doses of Hepatitis B (Hep B)
  • 2 doses of Hepatitis A (Hep A)
  • 1 doses of Varicella (Chickenpox)

OKLAHOMA STATE REQUIREMENTS FOR SCHOOL ENTRY (7TH-12TH)

  • Same requirements as for K-6th Grade
  • 1 Tetanus Diphtheria & Pertussis vaccine booster (Tdap)

Parents have the option to exempt their child from any vaccines with proper documentation. This form must be turned into the school nurse for submission to the Oklahoma State Department of Health for approval.
